The above information, including any accompanying or later submitted financial states or other documents, are supplied for the purpose of obtaining credit and the undersigned hereby certifies that the foregoing information is true and complete. The undersigned hereby authorizes Wasatch Container to investigate the references listed above as well as to conduct all necessary credit checks of the business and to verify information with banking institutions and trade references. Submission of this for also constitutes acceptance of the terms and conditions of the agreement.

If firm is less than one year old a financial statement must be attached to this application. A personal guarantee may be required.

Terms & Conditions

I agree to pay my account in accordance with your regular terms net 30 following purchase. I further agree to pay a late charge of 1.5% per month on balances not paid within 30 days of invoice. Should suit or action be instituted in collection of my debt to Wasatch Container, I hereby agree to pay Wasatch Container's attorney fees in a reasonable amount together with the costs and disbursements so incurred.
If suit is necessitated on behalf of Wasatch Container, applicant agrees that venue may lie in Davis County, Utah.
The undersigned hereby grants to Wasatch Container a security interest in all goods now or hereafter sold by Wasatch Container to the undersigned, including but not limited to (boxes, fabricated foam, wooden crates, cases, etc.) and all proceeds thereof (collectively, the "Collateral") to secure the payment and performance of the purchase price of the collateral and all amounts now or hereafter owing from the undersigned to Wasatch Container. The undersigned agrees to cooperate with Wasatch Container in executing any additional documents, instruments, financial statement, or amendments there to as Wasatch Container may request to perfect or continue the security interest granted herein. Upon a default by the undersigned of any of it's obligations to Wasatch Container, Wasatch Container shall have all rights and remedies of a secured party under the Utah Uniform Commercial Code, which rights and remedies shall be cumulative and not exclusive.
Applicant acknowledges receipt of a copy of this application and also agrees that as long as Wasatch Container extends credit to applicant, all terms and conditions of this agreement will be binding upon applicant. Wasatch Container may discontinue credit at time without notice.
I hereby certify that the above information and any attachments hereto, is correct and true to the best of my knowledge.
